Purchase Account: Your Authorized Purchase Account ("Account”) allows you, as an active member of the LACEA Insurance Services Inc., a wholly owned subsidiary of the Los Angeles Employees Association, Inc. (the "Association"), to order goods on your Account from Corporate Jewelers that willbe incurred on behalf of the Association by you as a purchasing agent. You may place orders up to your authorized purchase amount that has been set for you by the Association.

Member Status: You hereby acknowledge and agree that you are a current member of the Association. Further, you certify that this order, and any subsequent order(s), is made directly by you as the Account holder and that you have not given your Account credentials to ANY other party, including family or household members.

REPAYMENT ACKNOWLEDGEMENT
By clicking the "I Agree and Continue" button below, you (the "Member") are electronically signing this Repayment Acknowledgement and agreeing to the following:

Authorized Purchaser: The LACEA Insurance Services Inc., a wholly owned subsidiary of the Los Angeles Employees Association, Inc. (the "Association") has entered into an agreement with Corporate Jewelers whereby you are authorized to set up an Authorized Purchase Account ("Account") that will allow you to order goods on your Account, up to your authorized purchase amount which has been set for you by the Association, that will be incurred on behalf of the Association by you as a purchasing agent. Corporate Jewelers will invoice all orders placed on your Account directly to the Association according to the terms of the agreement entered into between Corporate Jewelers and the Association. Repayment Obligations:You hereby acknowledge and agree that you are solely responsible for repaying the Association for the order referenced above, and any future orders, pursuant to the terms set forth with you by the Association . Corporate Jewelers cannot and will not collect payment directly from you for any orders placed on your Account.
